Making a referral request

Make a change to a referral

We make every effort to refer you to an in network provider, and someone near your
address. However some providers may drop your plan or change l ocations without our
knowledge. Please keep i n mind t hat i t i s your responsibility t o make sure t he doctor or
facility i s contracted with your i nsurance before you get seen.
You can always verify by contacting t he office we referred you t o or by contacting your
insurance. Most i nsurance’s have a website where you can l ocate your own specialist
you want t o see. You will j ust need t o notify our office with either t heir f irst and l ast
name of t he provider or t heir group name and a phone number.
Please make sure our office has your correct address and phone number and email.
Along with t he correct i nsurance i nformation.
If we have provided you with a referral and you do not wish t o go t here t hen you will
need t o contact your i nsurance f or assistance i n l ocating a specialist t hat will best suit
your needs i e: hours, l ocations etc.
Please keep i n mind t hat referrals can t ake up t o 14 business days unless a provider i n
our office states otherwise.
If you are requesting a specialist outside of Maricopa County you will need t o contact
your i nsurance directly t o get assistance. Once you have t he name of t he provider
please call, email or message me f rom t he portal with t he f irst and l ast name and t heir
phone number so t hat I can send a referral and records t o t hem.
The easiest and quickest way t o reach me i s t hru email at referrals@cpfhc.com, or t hru
our online portal. I f you do l eave a voicemail I will get back t o you once t he referral has
been completed or i f I need additional i nformation.
